issue #6: not sure if we have to refer to BePatient in the logical model as we don't have a logical model for BePatient just yet
the branch is not yet merged (issue #1 branch) so some issues cannot be closed yet
only follow up advice for colon screening
implement issue 5 so that it can be closed - remove followup.approved as agreed last meeting
rename logical model to BePopulationHealth so that issue #4 can be closed
